Troubleshooting
I completed the SF-12 but nothing happened when I clicked Calculate.

Make sure all 12 questions have been answered. The progress bar at the top of the questionnaire should show 12/12. If any questions are missing, an error message will tell you which ones.

How are the PCS and MCS scores calculated?

The SF-12 responses are recoded to 0–100 scales, grouped into 8 health subscales, standardised against US population norms, and combined using published factor score coefficients to produce the Physical Component Summary (PCS) and Mental Component Summary (MCS) T-scores (mean = 50, SD = 10 in the reference population).

Can I change my SF-12 answers after calculating?

Yes. Click Edit SF-12 Responses below the score cards to reopen the questionnaire. Your previous selections will still be there. After making changes, click Calculate Scores again.

What does the highlighted decision path mean?

The highlighted boxes show the exact sequence of decision rules applied to your inputs. Each highlighted step represents a threshold in the decision tree that led to the final predicted cluster.

Is my information stored?

No. All scoring and prediction runs directly in your browser. Nothing is transmitted to a server. For privacy reasons, please do not enter names, MRNs, or other identifying information.
